County Council Funding aims to enable homeless people to get health and housing help
Derbyshire County Council is providing funding to a service that helps homeless people stay healthy and find housing.
The council is giving a grant of £20,000 to Derventio Housing Trust to support their Healthy Futures service.
Healthy Futures is a project managed by Derventio that works with homeless people who use local health services on a regular basis.
The Healthy Futures services acts as a bridge between hospital and community for people who have multiple needs that may include homelessness, mental health and drug and alcohol issues.
Healthy Futures aims to reduce repeat hospital admissions as well as support people to find appropriate accommodation when they leave hospital.
Derbyshire County Council’s Cabinet Member for Adult Care Councillor Jean Wharmby said: “We’re pleased to be able to support the Healthy Futures service with this grant.
“Homelessness comes with a great deal of challenges including access to appropriate health care and knowing where and when help is needed.
“People who are homeless are more likely than the general population to have poor health.
“The Healthy Futures team works with homeless people in hospital and they also work closely with people with complex, multiple needs to help them to improve their overall health and quality of life.”
Healthy Futures also helps to coordinate services so that people who are homeless and have multiple health problems can leave hospital as soon as possible as well as advising people on how to use health services effectively.
The service ensures that local support services, NHS treatment services, housing and support all work together to help reduce the inappropriate, and repeated, use of health services.
Healthy Futures works with people who are using the following health services:
• Hospital admission
• A&E
• Emergency ambulance
And who are either:
• Homeless with no permanent address of their own
Or
• People with multiple complex needs including a history of homelessness
